Test Background

Antibodies to the thyrotropin receptor (TSHR) have an important role in the pathogenesis of autoimmune thyroid disease. Stimulating TSHR antibodies result in hyperthyroidism and are a specific and sensitive marker of Graves’ disease. Blocking TSHR antibodies can be seen in Hashimoto’s hyperthyroidism where they contribute to hypothyroidism.

Measurement of TSHR antibodies is recommended for the diagnosis of Graves’ disease in patients with confirmed thyrotoxicosis (NICE NG145) and in Hashimoto’s thyroiditis with extra-thyroidal manifestations. High titres of TSHR antibodies during pregnancy can result in foetal hyperthyroidism.

This assay is performed by competitive ELISA and measures stimulatory and the less common inhibitory antibodies.

Indicated for the investigation of thyrotoxicosis and Graves'-affected pregnancy.

Additional tests may be added by discussion with the relevant laboratory. The time limit for adding additional tests to a sample already received in the laboratory will depend on the type of sample and the department it was sent to.
