Vancomycin

Monitoring vancomycin levels ensures dosing can be adjusted to achieve the desired therapeutic range.

Take a pre-dose (trough) vancomycin level for all patients on iv vancomycin before second maintenance dose. Record what time the sample is to be taken on the request form.

Pre-dose (trough)
10-15 mg/L Standard target level
15-20 mg/L Target level if severe or deep-seated infections. Such cases should be discussed with the infection team.
